Our Licensed Practical Nurse of Chronic Care performs duties concerned with developing and managing the Chronic Clinic Program to identify and control chronic illnesses.
Education for Licensed Practical Nurse:
- Graduation from an accredited School of Nursing
Experience for Licensed Practical Nurse:
- One (1) or more years clinic experience preferred
Licenses/Certifications for Licensed Practical Nurse:
- Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) within the state
- Current CPR certification
ResponsibilitiesDevelop and implement effective systems for identification, prevention and control of chronic illnesses.Monitor incidence of chronic, infectious and communicable diseases among inmates, including HIV/AIDS, TB, hepatitis, hypertension, pulmonary disease, and other conditions requiring regular monitoring.Monitor compliance with regulations and requirements pertaining to chronic illness.Promote a safe and healthy environment for the medical staff.Direct and conduct monthly chronic clinic activities, including auditing, scheduling appointments and medication compliance, Quality Improvement, and regular meetings.Ensure that referrals are provided for community follow-up and coordination of care after inmates are released from the facility.Confer with the Health Services Administrator to develop and conduct education and training programs for inmates and staff.Promote the development of positive social skills through modeling appropriate behaviors and intervening when inappropriate behaviors are observed.Maintain accountability of inmates and property; adhere to safety practices.It is expected that the incumbent shall perform other duties as assigned within his/her capabilities as determined by management.Must be able to obtain and maintain security clearance.Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.Must be alert at all times; pay close attention to details.Must be able to work under stress on a regular or continuous basis.Post orders, if applicable, per site contract.Perform other duties as assigned.
